Flames leapt from an upper floor window of the Burelli Street watering hole after the fire started in a storeroom about 3pm.
Wollongong police Inspector Brian Wyver said the three juveniles were arrested nearby.
Last night they were assisting police with their inquiries.
Keiraville woman Kylie Newman watched the drama unfold after she spotted smoke and flames coming from a third-floor window, moments before the window and its frame fell to the footpath.
She said fire crews arrived moments later and two firefighters wearing breathing apparatus ran into the building carrying a hose.
Fire and Rescue NSW Inspector Brendan Cox said the blaze had been contained to a single room in a derelict section of the third floor.
There were no reports of injuries.
